1. Watch the video above to learn how to tell your team about the program. Then, share this link with them to complete a survey about the practice.  This is optional but highly recommended.  We will create  summary two weeks from your sign up date and provide it to you.  We will not give you any verbatim responses as it is our goal that your team feel comfortable sharing and not have to worry about you, as the doctor, reading the responses.

2. Complete the Client Intake Questionnaire below:

3. Upload your most recent trailing 12 month profit and loss statement, as well as 5 screenshots of your schedule from 5 different days by using the form below. (we are looking at how you schedule and what an average day looks like, there is no need to find a perfect day.)

4.  Schedule your first coaching call 2-3 weeks from today (It is essential that all previous steps are complete and your team has completed all the surveys at least one full week before the call).  Click the button to schedule.  My EA, Maggie, will reach out and is there to help you with anything you need.
