Anonymous Team Survey

Hello Team Member, 

We are Dental Practice Heroes Coaching and your doctor has reached out to us to help make your practice a better place to work as well as for your patients to receive care.   We are beyond excited to work with you. 

At our core, we believe through open communication, collaboration, and coaching, that all issues and frustrations in a dental practice can be minimized or eliminated, so that everyone can enjoy and love the time they spend at the practice.  In order to serve you and your team, we need your input. 

This survey is anonymous.  It will not be shared with your doctor.  Nothing will identify you personally by completing it.  We will read over the responses, and share common themes that appear from the answers from you and your teammates. We will not share anything verbatim, we are only looking for common themes.  We need you to trust us when we tell you that, and that is not something we take lightly.  Your answers are safe.

Please use this opportunity to explain to us everything you can about your practice, so that we can come up with action plans to help remedy the issues.

We are not here to tell you how to run your practice.  We are here to help you have a workplace that fosters positive feelings and teamwork, so that you can feel good about your job and take the best care of your patients.
